Independent Support Coordination

Our goal is to make your NDIS experience stress-free. We do this by working from a person-centred approach. By actively listening and understanding your needs and communicating effectively at every step. We will play a key role in supporting you to navigate the complex NDIS system and build your knowledge and capacity to learn about NDIS. We will support you with the NDIS Planning and Implementation of support so you can get the most out of your NDIS Plan. We will assist you to connect to services and work towards your NDIS goals.

What is the role of a Support Coordinator?

A support coordinator helps participants in searching, coordinating, and negotiating with service providers of choice.

A support coordinator constantly communicates with participants to ensure progress and effective delivery of services as per the participant's goals.

How will we assist you?

Help you to understand your NDIS plan
Assist to implement your NDIS plan
Assist to connect you with providers of choice
Assist in building capacity and independence in order to coordinate services on your own if required
Help you to achieve greater independence to direct services and supports for the future
Strengthen your opportunities to participate in the community
Help you to understand Service Agreements with providers
Ensure that the supports you are connected to meet their obligations both legally and contractually
Manage any issues with your supports if they arise
Assist you with NDIS plan reviews
Provide the NDIS with reports as required
Help you to have genuine choice and control
We will work with you and your representative to make your NDIS experience as seamless as possible
